We ask that you fill out the transfer forms and bring it along with your current treatment paperwork so our team can make sure you take up right where you left off.

If you currently have a treatment center and treatment plan at another facility but would like to transfer to our clinic, we can resume your treatment right where you are in the process. It's very simple, you just fill out the transfer form provided on our website and bring it with you to your scheduled appointment.
